This Website, www.hidocsmatch.com, (hereinafter referred to as “Site” except where clarified otherwise) is the product of, and is provided by, PMAG. All use of the Site is subject to the Terms and Conditions set forth below.

By entering and using the Site, you acknowledge that you have read and understood such Terms and Conditions and that you agree to abide by them. Also, you agree that PMAG may use information that you supply through the Site in connection with services provided to you by PMAG. Legal information provided on the Site is intended to be informational not to replace consultation with an attorney. PMAG retains all copyrights and other proprietary rights in or relating to any content, including any software, provided on the Site. Except for downloading and printing a small number of copies of the content for your personal use only, you may not sell, transfer, reproduce, transmit, distribute, perform or display such content without the express consent of PMAG.

As part of the registration process, each member user will select a password (“Password”) and Login Name (“Login Name”). You shall provide PMAG with accurate, complete, and updated account information. Failure to do so shall constitute a breach of these Terms and Conditions, which may result in immediate termination of your access to the Site.

You may not:

1. Select or use a Login Name of another person with the intent to impersonate that person;

2. Use a name subject to the rights of any other person without authorization;

3. Use a Login Name that PMAG, in its sole discretion, deems inappropriate or offensive; or

4. Allow any third party to use your Login Name and/or Password.

 

You shall notify PMAG of any known or suspected unauthorized use(s) of your Account, or any known or suspected breach of security, including loss, theft, or unauthorized disclosure of your password. You shall be  responsible for maintaining the confidentiality of your password, and shall indemnify and hold PMAG harmless for and against any claims, actions or demands, liabilities, and settlements, resulting from, or alleged to result from, allowing third parties access to the Site utilizing your Login Name and/or Password, including but not limited to, alleged violations of the Health Insurance Portability and Accountability Act of 1996 and related  regulations (45 C.F.R. Parts 160 and 164) (known also as “HIPAA”) .  Any fraudulent, abusive, or otherwise illegal activity may be grounds for termination of your access to the Site, at PMAG’s sole discretion, and you may be reported to appropriate law-enforcement agencies.
